  1. Burning Sensation (Stomatopyrosis)
    The earliest and most common sign is a persistent burning sensation in the mouth, particularly when consuming spicy or hot foods. This occurs due to the initial inflammatory phase where the oral mucosa becomes hypersensitive. 
  2. Blanching of the Oral Mucosa
    Healthy oral tissue is pink and vascular. In OSMF, the mucosa begins to look pale, marble-like, or “blanched.” This “white-patch” appearance is a result of reduced blood flow and the early stages of collagen deposition under the skin. 
  3. Presence of Palpable Fibrous Bands
    As the disease progresses, you may feel thick, rope-like structures inside your cheeks or lips when running your finger along the inner lining. These are fibrotic bands that have lost their natural elasticity. 
  4. Restricted Mouth Opening (Trismus)
    This is the hallmark of advanced OSMF. The stiffening of the cheek tissues makes it difficult to open the mouth fully, eventually affecting your ability to eat, speak, and maintain oral hygiene. 
  5. Tongue Restriction and Depapillation
    In many cases, the tongue becomes smooth (loss of papillae) and its movement becomes restricted. You might find it difficult to protrude your tongue or notice a change in your speech clarity.

Management of OSMF: Medical vs. Surgical

Medical Management (Early to Moderate Stages)

Medical therapy focuses on slowing down the fibrosis and improving the vascularity of the tissues. 

  • Intralesional Injections: A combination of corticosteroids (to reduce inflammation) and Hyaluronidase (to break down the fibrous collagen) is often injected directly into the bands. 
  • Nutritional Supplementation: High doses of antioxidants, Lycopene, and Vitamin A/B-complex are prescribed to combat oxidative stress. 
  • Physiotherapy: Forceful mouth-opening exercises using specialized appliances or simple “ice-cream stick” therapy are crucial to maintaining the gains made through medication. 

Surgical Management (Advanced Stages)

When medical therapy no longer suffices and mouth opening is severely restricted (typically less than 20mm), surgical intervention becomes necessary.

  • Fibrotomy: The surgical release or “cutting” of the blanched fibrous bands to immediately increase the inter-incisal distance.
  • Laser-Assisted Release: Using advanced dental lasers for precise excision of bands, which results in less bleeding and faster post-operative healing. 
  • Grafting & Flaps: To prevent the released bands from scarring back together, the surgical site is often covered with grafts (like the Buccal Fat Pad or nasolabial flap) to provide a fresh, flexible lining.
